Lipid Optimisation Programme
The Lipid Optimisation Programme is delivered by Primary Care Alliance CIC in collaboration with Royal Berkshire Hospital. The programme works with GP practices across Berkshire West to identify patients who may benefit from a cholesterol and cardiovascular risk review. It supports evidence-based treatment, shared decision-making and consistent follow-up for people at increased risk of heart attack, stroke and other cardiovascular disease.
Overview
High cholesterol is one of the major modifiable risk factors for cardiovascular disease. Many people at increased risk can benefit from lifestyle advice, medication review and treatment optimisation.

Through collaboration between Primary Care Alliance CIC, Royal Berkshire Hospital and local GP practices, the programme supports a more joined-up approach to lipid management across Berkshire West.

The programme aims to help practices identify patients with established cardiovascular disease, whose cholesterol management may need review.
How the collaboration works
Primary Care Alliance CIC supports practice engagement, patient identification, recall, review processes and delivery across primary care.

Royal Berkshire Hospital provides specialist lipid expertise and support for more complex cases where additional advice may be needed. The programme works alongside practices and uses the GP record as the main clinical record.

Why have I been contacted?
You may be contacted because your GP record suggests you could benefit from a cholesterol or cardiovascular risk review.
This does not necessarily mean anything is wrong. It means your records show that a review may help reduce your future risk of heart disease, stroke or related conditions.
The programme is delivered in collaboration with Royal Berkshire Hospital, but in most cases your review will remain part of your primary care record and will be shared with your GP practice.
For practices and clinicians
The Lipid Optimisation Programme supports practices with proactive identification, review and optimisation of patients who may benefit from lipid management. The programme is delivered by Primary Care Alliance CIC in collaboration with Royal Berkshire Hospital, combining primary care delivery and specialist lipid expertise.
The programme can support:
- Search and recall of eligible patients
- Review of patients with established cardiovascular disease
- Identification of patients above lipid targets
- Review of patients on no lipid-lowering therapy
- Review of statin intolerance, contraindications or previous declines
- Consideration of treatment intensification where clinically appropriate
- Coding quality and audit support
- Follow-up monitoring and outcome reporting
- Escalation or advice for more complex lipid cases
